CREDIT CARD AUTHORIZATION FORM
To secure your event date, a credit card hold is required. The card will not be charged unless the event is not cancelled, rescheduled or modified in accordance our Event Cancellation/Reschedule Policy. PLEASE NOTE: Your date is not confirmed until this credit card authorization form is submitted.
EVENT CANCELLATION/RESCHEDULE POLICY: If the event is cancelled, rescheduled or modified seven (7) days prior to the event date, the client will incur no charges. If the event is cancelled, rescheduled or modified less than seven (7) days 50% of the contracted bay rental costs will be charged. If the client no shows for the event, then the client is responsible for 100% of the contracted bay rental cost. All cancellations and modifications must be made in writing to amy@swingzonegolf.com
Payment in full is due immediately following the event.
